Fundamentals of Human Error and Resilience (HEAR)

The benefit of every employee understanding the language, concepts, and principles Human Error and Resilience cannot be overstated. This understanding is foundational to building a culture where the workers shift from being a problem to be solved into an informed group of problem-solving allies. It covers topics related to the “facts” of human error, and how to recognize “error-likely situations”. Understanding the strategy of human error reduction and how to minimize the consequences of human error creates resilience within your organization. Group activities will demonstrate how human errors are not just the product of individuals, but also the unique types of errors produced by groups working together. This 8 hour course includes “ah-ha” moments designed to demonstrate the immediate application of HEAR tools and processes as its objective.

Intermediate Application of Human Error and Resilience (HEAR)

Beyond the fundamentals; this course is designed for those who direct and lead work at the line level. Ensuring that these leaders know how to evaluate work processes for error likely situations and to apply error reduction and resilience into how the work is done with the goal of reducing the impact of human error is the overall objective of this course. After a review of the HEAR fundamentals at the beginning of the course, the remaining time will be spent on applying the concepts and tools in your real-world job situations. Students will be presented with the concepts of critical steps and risk important steps and discuss scenarios on how to evaluate the potential for unwanted outcomes. Once these concepts are understood, the participant will be challenged with choosing error reduction tools to use and how to apply them.

The guide for this two-day (16 hours) course is the second Department of Energy manual “Human Performance Tools for Individuals, Teams and Managers”. Prior completion of the 8 hour “Fundamentals of Human Error and Resilience (HEAR)” course is strongly encouraged and will prepare the students to get the most out of this course, but it is not required. There is a “fundamentals” review at the beginning of this course, but not to the depth of the one-day fundamentals course. This is an interactive course and those students who actively participate will receive the best preparation for applying the concepts and principles.

The Fundamentals of Using HEAR Tools

“Everything looks like a nail when you have a hammer in your hand”. Not every tool will be used in every situation. Understanding how to choose and apply specific tools to the problem at hand is the subject of this one-day (8-hour) course. Specific scenarios will be role played to demonstrate the effectiveness of some of the more common error reduction tools. This course is designed to train the company trainers on how to introduce the use of error reduction tools into “how we do work”. This course is designed for those who have attended the “Intermediate” training and are ready to implement tomorrow. Highly engaging and interactive exercises require active participation from all attendees.

This encounter is a follow up to our initial consultation and is intended for individuals in decision making positions. It is tailored to your specific industry and application. It provides an overview of the Human Performance Improvement (HPI) discipline in a fast-paced format. Specific measurable results and impact are discussed. Attendees will participate in activities that demonstrate the HPI tools and defenses that reduce error and add resilience to your operations. Time is allotted for Q&A.

Keynote Address

Keynote addresses are meant to briefly communicate the basic idea of Human Performance Improvement and the HEAR application. It is presented in a humorous yet thought-provoking way with specific application to the audience. It introduces a very positive approach to human fallibility. Attendees will walk away with a tool or two to immediately put into practice and a lot to think about.
